How long does orthodontic treatment take? A specialist guide for adults and teenagers

One of the first questions I’m asked in clinic is: “How long will treatment take?” It’s a completely fair question — you’re planning work, family life, events, and you want a result that looks great and lasts.

As a UK-registered Specialist Orthodontist, my job is to plan treatment that’s safe, efficient and stable long-term — not rushed. Below is a clear guide to typical timelines and the factors that genuinely affect treatment length.

Typical orthodontic treatment time

Most orthodontic treatment takes between 12–24 months. Some cases are shorter, and some are longer — the right timeline depends on what we’re trying to achieve and what’s needed for a stable bite.

  • Minor alignment: often around 6–12 months
  • Moderate alignment + bite improvement: often 12–18 months
  • More complex bite correction: often 18–24+ months

What actually affects how long treatment takes?

Treatment time is not just about “straight teeth”. It’s usually about tooth position, bite function, and stability. These are the biggest factors:

  1. Complexity of tooth movement. Rotations, spacing, crowding and impacted teeth can take longer than simple alignment.
  2. Bite correction. Improving how the teeth fit together (overbite/overjet/crossbite) can add time — but it’s often what protects the result.
  3. Growth and timing (teenagers). For some teenagers, timing treatment with growth can make things more efficient. In certain cases, functional appliances may be used as part of a wider plan.
  4. Your wear time (aligners). Clear aligners work best when worn as prescribed (typically around 22 hours/day). Consistency matters.
  5. Appointments and breakages. Missed appointments, broken brackets, or lost aligners often slow things down.
  6. Tooth and gum health. Healthy gums support safe movement. If gum health needs improving first, it’s always better to stabilise that before pushing ahead.

Do braces take longer than aligners?

Not necessarily. The “fastest” option is the one that matches your clinical needs.

  • Clear aligners (e.g. Invisalign® / Spark) can be very efficient for many adult cases — especially when wear time is excellent.
  • Fixed braces (e.g. Damon braces) are often the most reliable option for certain types of bite correction and complex movements.
